A single digit input error on behalf of Qxeni Telecomm employee in the UID field and an improperly configured mail scanner resulted in the deaths of over 10,000 individuals and a cost to the Rimworlds to the tune of billions. ArQivist followed exact protocols. No disciplinary action recommended. Sanctions request forwarded to ArQive Department of Legal Counsel. Reprisal Action pending review.
